Following the keynote at the San Antonio Medical Foundation, several additional questions came in. They covered a wide range of topics, including emotional regulation, sleep, caffeine, relationships, and environmental factors.
At first glance, these questions appear unrelated. They are not.
Each one reflects the same underlying condition: how to continue performing when capacity is not unlimited, and the demands of the environment are not decreasing.
One attendee asked, “Thoughts on caffeine?” Another asked, “Recovery time (sleep) is key for reducing stress, but what recommendations do you have for someone who struggles with elevated heart rate overnight?” Someone else asked, “How do you handle stressful situations when your business partner is also your spouse, especially if both of you are facing the same pressures simultaneously?”
Different situations, but the same constraint shows up in each one. Capacity is being used at a high level, and the question becomes how to maintain decision quality without reducing performance or increasing strain.
Emotional Regulation vs. Aloofness
One question captured an important leadership distinction: “How can you distinguish between emotional regulation and aloofness?”
This distinction becomes more important as pressure increases. Emotional regulation helps you manage your internal responses while remaining present and connected to others. Aloofness, by contrast, reduces input by creating distance.
The difference is not how it feels internally, but how it is experienced externally. When regulation is effective, others experience steadiness. As distance increases, however, others often feel disconnected.
Shared Pressure in Close Working Relationships
Several people asked about managing stress when a business partner is also a spouse, or when both individuals are facing the same pressures at the same time:
“How do you handle stressful situations when your business partner is also your spouse, especially if both of you are facing the same pressures simultaneously?”
Shared conditions do not mean equal capacity or identical processing needs. When both individuals attempt to process everything together in real time, it often increases cognitive and emotional load for both. Separating how and when processing happens, and reconnecting once capacity is more stable, tends to produce clearer communication and better decisions. In other words, use outside resources to process and recover. Talk to friends, seek out a mentor, tap into people in your community, or engage a mental health professional or coach to help you through.
Supporting Transitions
Questions about supporting children during major transitions also came up:
“Any tips on how to handle our children’s stress as they transition from high school to college? Or college to career?”
Transitions increase uncertainty, and uncertainty increases cognitive demand. This often shows up as hesitation, frustration, or inconsistent behavior.
What is most effective in these situations is not removing the stress, but reducing unnecessary decisions and adding structure where possible. This stabilizes capacity during a period when demand is naturally higher. In other words, take things in small bites. Talk about what your student is feeling and make room for some unexpected reactions.
Caffeine
One attendee asked directly, “Thoughts on caffine?”
Caffeine is often used to maintain performance. It can temporarily increase alertness, but it does not restore capacity.
When caffeine is used to override fatigue signals, it can delay recovery and increase strain later in the day or over time.
We all react differently to caffeine, too. Some people can drink coffee or energy drinks and work well. Others get jittery or anxious. For some of us, like me, who have heart arythmias, caffinne can trigger an episode. Pay attention to the signals your body is sending!
Environment and Indoor Lifestyles
Another question focused on the environment: “What are your feelings about stress being amplified by our indoor lifestyles vs getting outside to breathe fresh air?”
Indoor environments can increase cognitive and sensory load. Artificial lighting, constant input, and limited variation in the environment can keep the system in a more continuously engaged state. By contrast, being outside tends to reduce that load. There is less input to process, fewer competing demands, and the system has more opportunity to downshift.
So yes, getting outside can support capacity.
But it is important to be precise about what it does and what it does not do.
It does not offset sustained demand. It does not restore capacity if the underlying system load remains unchanged. And it does not replace the need to manage how much demand you are carrying day to day.
In high-demand environments, the environment is a supporting factor, not a primary driver.
The more useful way to think about it is this: getting outside can help reduce baseline load and create conditions that make recovery more likely. But the primary work is still managing demand so that your capacity can stabilize and decision quality can be maintained.
Sleep and Elevated Heart Rate
Sleep-related questions reflected a different type of signal:
“Recovery time (sleep) is key for reducing stress but what recommendations do you have for someone who struggles with elevated heart rate overnight?”
When the system does not fully downshift overnight, it suggests that the demand placed on it during the day has not been fully resolved.
Reducing late-day cognitive and emotional load and creating a clearer transition out of work conditions can support a more complete recovery. When this pattern persists, you should probably have a conversation with your doctor.
Cortisol and Chronic Stress
Another question asked, “Can your techniques lower cortisol from chronic stress?”
Yes. And it is important to be direct about that.
When cortisol remains elevated for extended periods, it is not just a marker of stress. It becomes part of the problem. It affects sleep, recovery, cognitive clarity, and decision quality. If it stays elevated, the system does not fully reset, and capacity continues to erode.
So the goal is not to ignore cortisol and hope it resolves on its own.
The goal is to actively bring it down while also addressing the conditions that are driving it up.
That requires two things working together.
First, you need to reduce cortisol in the moment so the system can downshift. Without that, recovery remains incomplete, and capacity does not fully restore.
Second, you need to address the sustained demand that is keeping the system activated. If that demand remains unchanged, cortisol will rise again.
This is not an either/or approach.
It is a sequence and a system.
Lower the physiological load so the system can recover. Then adjust the demand so that recovery can hold and decision quality remains stable.
Recovery Without Additional Disruption
“Could you share some stress relievers that don’t cause excessive distraction?”
That question gets at something important. In high-demand environments, the goal is not to step away completely. It is to reduce load without losing continuity.
The most effective recovery tools in these conditions are not dramatic resets. They are low-friction adjustments that allow the system to downshift while you remain oriented to your work.
Here are a few that tend to hold up in real environments:
1. Controlled breathing without stopping work
You do not need a full session. Slowing your exhale for a few cycles while reading, listening, or between decisions can begin to bring the system down. No transition required.
2. Narrowing your field of attention
Instead of switching tasks, reduce input. Close extra tabs. Silence nonessential notifications. Stay with one decision at a time. This reduces cognitive load without pulling you away.
3. Physical downshift without disruption
Small physical adjustments can signal the system to settle. Lower your shoulders. Unclench your jaw. Sit back for a moment instead of leaning forward. These are subtle, but they change the physiological state.
4. Clear one open loop
If your attention is scattered, identify one unresolved item and close it. Not five. One. That small completion reduces cognitive load and restores usable capacity.
5. Short environmental shift with a defined boundary
Step outside or change rooms briefly, but with a clear endpoint. The purpose is not escape. It is to reduce input and allow a reset without losing momentum.
What these approaches have in common is that they do not require you to stop, plan, or transition into something else. They reduce demand inside the work, rather than pulling you out of it.
If a recovery strategy requires coordination, scheduling, or sustained attention, it will often compete with capacity instead of restoring it.
The standard to use is simple:
Does this reduce load, or does it add another layer of effort?
The more it reduces load without interrupting continuity, the more effective it will be under sustained pressure.
Closing Perspective
Each of these questions reflects a different entry point into the same challenge.
Not how to avoid stress. Not how to reduce effort.
But how to continue performing in environments where demand remains high, without allowing that demand to reduce capacity below what the work requires.
That is the work.
Protecting capacity so that decision quality remains stable, even as conditions continue to shift.
