When Decisions Don’t Close: Managing “Shoulda, Coulda, Woulda”
There is a specific pattern that appears in high-demand environments, one that people often describe but rarely define clearly.
A conversation that continues to replay.
A decision that feels finished but does not stay that way.
A moment that keeps pulling your attention back, even as new decisions are required.
At the San Antonio Medical Foundation, one of the questions captured it directly: “How do you manage shoulda coulda woulda?”
Most people experience this as overthinking.
A more accurate way to understand it is as a capacity signal.
What This Signal Indicates
When a decision does not reach a clear endpoint, the brain continues to process it in the background. It replays the situation, evaluates alternative outcomes, and tests different versions of what could have happened.
At the same time, new decisions are still being made.
This creates an open loop, which draws on capacity.
The signal is not that something is wrong with how you are thinking. The signal is that capacity is being used by past decisions that have not fully closed.
Why This Increases Under Sustained Demand
Under sustained pressure, decisions are made more quickly and often with incomplete information. In many cases, there is no clear right answer, only a reasonable one based on what is known at the time.
When conditions do not allow for certainty, the brain has a harder time recognizing a decision as complete. It continues to scan for better alternatives, not because the decision was poor, but because the conditions did not allow for full resolution.
As demand continues, more decisions remain partially open.
The number of open loops increases.
Capacity is reduced.
The Impact on Decision Quality
The effect is rarely immediate or obvious.
Work continues. Responsibilities are met.
But attention is no longer fully available to the present moment. Part of it remains tied to what has already happened.
That division of attention increases cognitive load and reduces usable capacity for current decisions. Over time, this shows up as slower thinking, increased friction, and more effort required for decisions that would normally be straightforward.
The issue is not the original decision.
It is that the decision is still consuming capacity.

One participant described replaying a routine conversation well after it ended. There was nothing particularly complex or high-stakes about it, but there was no clear internal conclusion.
By the time they moved into the next meeting, part of their attention was still tied to what had already happened.
Nothing had gone wrong.
But capacity had already been reduced.
How to Close the Loop
If this pattern is a capacity signal, the response is not to suppress it. The response is to complete the decision.
A decision closes when it has a clear evaluation point.
A practical way to establish that is to ask:
“Was this decision reasonable based on the information available at the time?”
That question aligns the evaluation with the conditions in which the decision was made.
If the answer is yes, the decision is complete.
If the answer is no, identify what information was missing, determine what would change next time, extract that learning, and then close the decision.
In both cases, the loop ends.
Capacity is released.
Why This Matters
This is not about eliminating reflection or avoiding learning.
It is about recognizing that open loops are a signal and a source of capacity loss.
When those loops continue, they quietly reduce the capacity available for current work. Over time, that affects not just how much you can do, but how clearly you can think.
